Terrill Consulting can offer facilities the opportunity to have their current therapy programs reviewed for estimated staffing needs, profitability and potential contract negotiations. If a client is considering switching to an in-house program, renegotiating contracts with an existing contract agency, is considering a change in therapy providers, or simply wants a review of current utilization, Terrill Consulting can produce a comprehensive summary of options for a facility.   Staffing needs are based on historical data and will be analyzed at both conservative and ideal ratios giving the minimally required and a best case scenario.   Staffing costs will include area specific ranges, recruitment and management expenses for in-house programs. Cost savings or increased expense estimates for contract comparisons.   Expected utilization review will compare current therapy utilization with national averages as well as aggressive client/ conservative client averages within our customer database. Recommendations will be made based on facility demographics and administrative philosophy.   Our review considers facility specific factors in determining staffing needs, staffing costs, and expected utilization. As Terrill Consulting offers no staffing options itself, a client can rest assured recommendations are based on need and not the availability or profitability of our resources or services.
This review gives facilities the opportunity to have a historical analysis of Medicare RUG distribution. Terrill Consulting will consider multiple areas including: therapy utilization, nursing complexity and ADL scoring. To offer a comprehensive summary of how a facility’s utilization of categories compares to national averages, aggressive service providers and buildings of similar demographics make up.   Terrill Consulting can utilize this analysis to assist facilities in targeted in-servicing and if desired ongoing consultation to improve utilization of RUG categories to facility specific goals for identification of resident abilities and care which will lead to the highest appropriate reimbursement.   Terrill Consulting is committed to the belief that by accurately assessing and treating resident conditions, in the most liberal fashion a resident is entitled to, facility reimbursement will be ample enough to cover any costs incurred in providing exceptional care.
Terrill Consulting prides itself on a unique approach to reimbursement consulting. While traditional consultants may review current billing practices to identify inconsistency or integrity issues and use worst case scenarios to frighten an entity into utilizing their services, Terrill Consulting is more forward looking.   As previously mentioned, education is an important factor in management. Our concept is to hold staff accountable for knowing each individual beneficiary. Through this comprehensive multidisciplinary assessment of the person the team discusses the best approach to providing the highest level of care and predicts the appropriate reimbursable category for each individual. On a weekly basis the care and condition is reviewed and documentation recommendations are made. In cases where the reimbursable category predicted is not met the team determines if appropriate care was provided and documented at the predicted level. If it was provided as predicted the category is adjusted to match the care provided. In cases it was wrongly predicted Terrill Consulting determines if the error was an unpredictable change in condition or if department specific reeducation is needed to better asses residents in the future is provided.   Utilizing Terrill Consulting's method of consultation the resident remains at the heart of the reimbursement system. Facilities are reimbursed based on appropriate intervention and work to bring documentation and systems into line with superior resident care instead of instituting a rigid system and hoping improved resident care will follow.
Terrill Consulting offers its clients the additional value added service of managing audits, ADR documentation reviews and denial management. While these issues traditionally are handled by contracted therapy companies many of our clients prefer objective 3rd party involvement as these types of claims can be time consuming and costly if mishandled. The goal of the audits are to ensure compliance to regulations and appropriate reimbursement. The audits generally consist of initial data review followed by an on-site assessment. At the completion of the audit a report of the findings is compiled and provided to the client.
Terrill Consulting deeply believes in the profit of education, this includes both to individuals and to facility’s team members as a whole. In the current state of healthcare most facilities have encountered: numerous tagged surveys, Medicare audits and Medicaid audits which may not have ended favorably for the facility. As we are all aware, this is only the beginning. As the Healthcare field continues their cuts it is the resident and the facilities that provide exceptional care that appears to feel the result the most. Terrill Consulting has the ability to assist and implement forms that follow both federal and state’s most recent standards with the hope to be proactive and compliant. Terrill Consulting would also cater to each facility’s particularized needs from individual training, group in-services on nursing, administration and emergency topics, policies and procedures as well as implementation, and even completing mock surveys. This service could be used in an emergency situation or as an on-going service to better your staff, cost and care to your customers.
Terrill Consulting provides ADL coding education for facility staff. One of the goals for Terrill Consulting is to make services customizable and accessible to facilities.  Depending on the needs of the facility the service provided can include comprehensive in service with all educational materials provided or be customized form an a la carte menu of options. The ADL education provided designed to empower staff with the basic working knowledge of ADL coding and restorative programing  to improve documentation. Staff documentation that accurately reflects resident condition and care provided is essential in planning of care.
